DBQ: Link to Index of DBQ/Exams by Disability for DC 7620
Definition
The ovary is one of two glands in the female reproductive system that produces the reproductive cell and the hormones, estrogen and progesterone. Atrophy is a wasting or decrease in size of an organ or tissue.
Etiology
Causative factors related to hormonal atrophy of the ovaries include: menopause; malnutrition; anatomic abnormalities; endocrine disorders; or genetic defects. In addition, chemotherapy, and radiation of the pelvis may also cause ovarian atrophy.
Signs & Symptoms
There may be absence of menstruation or infertility. Hot flashes, excess perspiration, fatigue, irritability, insomnia, depression, memory loss, inability to concentrate, headache, anxiety or nervousness may be evident. There may also be dizziness, palpitations, nausea, changes in bowel and bladder function, and changes in the lower genital area.
Tests
Complete physical examination, papanicolaou (PAP) smear, and mammogram may be done prior to treatment. Laboratory studies may also be indicated.
Treatment
Oral or transdermal vaginal hormone replacement therapy (HRT) may be given. In certain genetic disorders, surgery may be indicated because of the possibility of cancer occurring.
Residuals
Possible development of heart disease, stroke, and osteoporosis may increase. There is an increased potential to develop endometrial cancer in women who have a uterus and who take estrogen replacement therapy. Certain genetic disorders may result in cancer. Ongoing physical examinations and mammograms may be necessary. Counseling, psychotherapy, and support groups may be indicated to assist the patient in dealing with the diagnosis.
Special Considerations
- None.
Notes
- Natural menopause, primary amenorrhea, and pregnancy and childbirth are not disabilities for rating purposes. Chronic residuals of medical or surgical complications of pregnancy may be disabilities for rating purposes.
- When evaluating any claim involving loss or loss of use of one or more creative organs or anatomical loss of one or both breasts, refer to 38 CFR 3.350 to determine whether the Veteran may be entitled to special monthly compensation. Footnotes in the schedule indicate conditions which potentially establish entitlement to special monthly compensation; however, almost any condition in 38 CFR 4.116 might, under certain circumstances, establish entitlement to special monthly compensation.
- May be entitled to special monthly compensation where the veteran has a single service-connected disability rated as 100% with additional service-connected disability or disabilities independently ratable at 60% or more, which are separate and distinct from the 100% service-connected disability and involves different anatomical segments or bodily systems. See 38 CFR 3.350(i)(1) – Special Monthly Compensation (SMC).
