6022 - Lagophthalmos

Definition
Lagophthalmos is a condition in which there is incomplete closure of the eyelids when an attempt is made to shut the eyelids.
Etiology
Etiologic factors include: injury to the facial nerve (7th nerve); a contracted scar on the eyelid; lack of tone in the orbicularis oculi; or exophthalmus.
Signs & Symptoms
Lack of closure may result in injury and irritation to the conjunctiva and cornea (see diagnostic code: 6018 conjunctivitis, other, chronic and diagnostic code: 6001 keratopathy). Signs and symptoms may include: redness; tearing; exudation; edema of the conjunctiva around the cornea; a sensation of a foreign body; a tender preauricular lymph node; pain; and photophobia.
Tests
Tests that may be done include: fluorescein staining; slit lamp; visual acuity; visual fields; ophthalmoscopy; and ultrasonography.
Treatment
The cornea and conjunctiva need protection from exposure, especially at night. Use of artificial tears and artificial tear ointment may be recommended.
Residuals
A complication such as keratoconjunctivitis sicca may develop from unprotected, dry eyes.
